As of January 7, 2025, all 2024 W-2s have been mailed to all associates who have worked for PDQ Staffing in 2024. They have been mailed to the most recently updated address we have on file for each associate. If you have had a recent change of address, or did not receive your W-2 by mail, please complete and submit the form below.

  1. Consent for Delivery Method
    By selecting a delivery method, you consent to receiving your W-2 form for the current tax year in the manner specified below:
    Email Delivery: If selected, you authorize PDQ Staffing to send your W-2 form to the email address you provide. The W-2 form will be encrypted and may require a password for access.
    Mail Delivery:     If selected, you authorize PDQ Staffing to send your W-2 form to the mailing address you provide.

  2. Email Security Notice
    If you choose email delivery, you acknowledge and agree that while PDQ Staffing will take reasonable steps to ensure the security of your information (e.g., encryption), email is not entirely risk-free. You accept any risks associated with electronic transmission.

  3. Contact Information Accuracy
    You are responsible for ensuring the accuracy of the contact information (email or mailing address) provided in this form. Incorrect information may result in delays or non-delivery.

  4. Withdrawal of Consent
    You may withdraw your consent for email delivery at any time by contacting PDQ Staffing at w2requests@pdqstaffing.com. If you withdraw consent, a paper copy of your W-2 form will be mailed to your address on file.

  5. Retention of Consent
    Your consent to this agreement applies only to the delivery of your W-2 form for the tax year requested and does not constitute a permanent delivery method preference for future years.

  6. Acknowledgment of Receipt
    PDQ Staffing will send a confirmation once your W-2 has been delivered to your selected method. If you do not receive your W-2 within 5 business days after this confirmation, it is your responsibility to notify a PDQ Staffing representative.
